Entry forms are available at all participating Bass Pro Shops locations. In order to enter the sweepstakes, take and complete official entry form or write down your name, complete address, daytime phone number, email address and date of birth on a 3”x5” piece of paper and then deposit it in an official ballot box at one of the participating Bass Pro Shops locations.

Online: Visit basspro.com/FreedomDaysSweeps and complete the official entry form with all required details.
