If you purchased a Surfer Boy frozen pizza product and found a unique code within the box, you can enter using your unique code by visiting SurferBoySweeps.com, and following the onscreen instructions to submit your entry. Each unique code can be submitted one time only. You will earn 1 Online Entry in the Sweepstakes per unique code. There is a limit of up to 8 Online Entries per person.
• Mail-in method (No Purchase Required): Write your full name, complete mailing address, valid email address, and telephone number on a piece of paper, insert that paper in a #10 business size envelope, affix postage, and mail via USPS First Class mail to: Surfer Boy Pizza Sweepstakes Entry, P.O. Box 157, Palatine, IL 60078-0157. Each Mail-in Entry must be written on a piece of paper and only 1 Mail-in Entry per envelope. Limit up to 8 Mail-in Entries per person.
