There are two ways to get entry in to the survey sweepstakes.

Survey: During the promotion period, visit participating Sears Hometown store, Sears Hardware store, Sears Home Appliance Showroom or Sears Outlet store location and make a purchase to get unique multi-digit code available on your cash register receipt. After that, visit www.searshometownfeedback.com, www.searshardwarefeedback.com, or www.searsoutletfeedback.com and first enter 12 digit sales check number and date of purchase to begin the survey. Give genuine answer of all questions regarding store. Upon completion of the survey, submit entry form with all required detail like name full name, complete physical address, daytime phone, number, and e-mail address, etc and get one entry in to the sweepstakes.

Additional Survey Section: Upon completion of the online survey, you will get another option to complete additional section of the survey. Entrants that are prompted must complete the extra survey section in its entirety to get one additional entry into the Sweepstakes.

On or about November 6, 2017, winner will be selected randomly among all eligible entries received in sweepstakes.
