- 2 photographs (1 full-face headshot and 1 full-length body shot in required format)

If you want to attend the casting call, then note down following three casting call locations:

- Los Angeles, CA at the SLS Hotel on August 18th 2012 from 1-4 PM PT
- Chicago, IL at the W Lakeshore Hotel on August 25th 2012 from 1-4 PM CT
- New York, NY at the Andaz Hotel on September 8th 2012 from 1-4 PM CT

During the casting calls, your photos will be taken and submitted with your completed entry form.

From October 9th to November 6th 2012, users can cast vote to their favorite finalist. Note down that one user can vote just one time per day.
