In second way, visit http://nestle.promo.eprize.com/confectionssweeps, and complete the entry form by providing required information such as name, address, day and evening phone numbers, email address, and date of birth, then submit it during promotion period.

In third way, write name, address, day and evening phone numbers, email address, and date of birth on a 3" x 5" piece of paper and mail it in an envelope with proper postage to: “The 3D Home Theater Sweepstakes,” c/o Dept. #574285, P.O. Box 5046, Kalamazoo, MI 49003-5046.
