Visit www.militaryhero.com and fill up entry form by providing your name, address, email address and phone numbers. You must login to your facebook account first, after then, you will get entry page. With the entry form, you have to upload either video that is no longer than 5 minutes, photo essay or essay up to five hundred (500) words which is written under the title of 1) What “being an American Hero” means to the nominee and 2) My nominee is a true American hero.
