3, By mail: Legibly hand-print, on a 3″x5″ paper, your name, address, birth date, phone number, and (optional) email address and mail it, along with a self-addressed stamped envelope, in a #10 envelope with proper postage affixed to: Award Headquarters, “LG G3 Open Beta Sweepstakes”, P.O. Box 365, Plymouth, FL 32768- 0365.
