Help Health Action Lab in such medical research to analyze the quality of care for patients from a patient-centered perspective, health plans, medical groups and doctors. Share your experiences that you personally faced during your treatment to Health Action Lab in the American Health Sweepstakes. That’s taking just few minutes. Your experiences and feedback will be helping other Americans make better test and treatment decisions. And opportunely, your entry into the sweepstakes could be included into the drawing where one lucky winner will get their choice of any one prize below valued at $500! Within the duration of the entry, visit healthactionlab.com and proceed to the relevant survey module. At the successful completion of the survey, you will be automatically included into the drawing. If you don’t want to take survey, then you have option to be part of the drawing by mailing.
