QR Code entry:

Scan the QR code in order to obtain the entry link and complete the online entry form and receive one entry into the sweepstakes.

Mail-in entry:

On a 3 x 5 paper hand print your full name, complete address, telephone number, email address, and date of birth and mail it to:
