4.Answer survey questions regarding the shopping experience
5.Submit the survey

At the end of the survey, the customer will be entered into a sweepstakes drawing where they could take home a $500 gift card that can be used at any Bass Pro Shops.

How to enter:

1) With Purchase:

If you have recently made purchase at a participating Bass Pro Shops retail store, you will receive an invitation printed on the receipt to participate in a survey. Visit bassprosurvey.com and complete the Survey within 7 calendar days from the date of purchase.

Once you have completed the Survey, you will be asked to enter the Promotion. If yes, get your registration first with full name, valid e-mail address, complete street address (P.O. Boxes are not accepted), city, state/province, ZIP/Post Code, telephone number and birthday. Then, click on the submit button to receive one entry into the Sweepstakes Period.

Limit: One entry per receipt.

2) Without Purchase

For those who have not made a purchase, go through via this entry method. During the Promotion Period, make your visit at survey.foreseeresults.com/basspro?flow=SS&locale=en, complete your registration and receive an entry into a Sweepstakes Period.
